This Notice describes how medical and personal information about you may be used and disclosed and how you can access this information.

 

How We Use and Disclose Information

CTBF uses and discloses health-related and personal information only for authorized program purposes, including:

  • Care coordination and support services

  • Education and workforce development programming

  • Required reporting to Texas Health and Human Services (HHSC)

  • Compliance, auditing, and quality improvement

We do not sell your information.

We do not use your information for fundraising or marketing without authorization.

You have the right to:

  • Request access to your information

  • Request correction or amendment

  • Request restrictions on certain uses or disclosures

  • Request confidential communications

  • Receive an accounting of disclosures

  • File a complaint without retaliation

Complaints

If you believe your privacy rights have been violated, you may file a complaint with CTBF or directly with the U.S. Department of Health and Human Services. Filing a complaint will not affect your services.

Central Texas Birth Fund is a nonprofit organization registered with the Texas Secretary of State.

Registration does not imply endorsement by the State. Our federal 501(c)(3) tax-exempt status is pending.
